Who Can File Form-8

1. Can be filed only by a person whose name is already included in the electoral roll seeking correction to his/her particulars already printed in the electoral roll.

When Form-8 can be filed

1. Form-8 can be filed for correction to particulars of a person entered in the electoral roll. The application can be filed after draft publication of electoral roll of the constituency.
2. Only one copy of the application is to be filed.

3. The Application can also be filed for correction of particulars relating to oneself in the finally published electoral roll when the revision programme is not going on. During non-revision period, application must be filed in duplicate.

Where to file Form-8

1. During revision period, the application can be filed at the designated locations where the draft electoral roll is displayed as well as the Electoral Registration Officer and Assistant Electoral Registration Officer of the constituency.

2. During other period of the year when revision programme is not going on, the application can be filed only with the Electoral Registration Officer.

How to Fill up Form-8

1. Visit the CEO Andhra Pradesh website and Click E-Registration->Assembly Constituency.
2. Then Select Form 8 (Correct your Card) -> Link 1

3. Fill the required details to complete your Registration,

Step 2 : Select Assembly Constituency *
Step 3 : Upload Supporting Document
i. Your Photograph*
ii. Age Proof
iii. Address Proof

Step 4 : Enter Applicant’s Details
Step 4.1 : Name*

Step 5 : Details of registration with electoral roll
Step 5.1 : Enter Part No.of Electoral Roll*
Step 5.2 : Enter Serial No.of Electoral Roll*
Step 5.3 : Enter EPIC No.(if issued)

Step 7.5 : Select Gender*
Step 7.6 : Select Type of Relation*
Step 7.7 : Enter House/Door number*
Step 7.8 : Enter Street / Area / Locality*
Step 7.9 : Enter Town/Village*

Step 7.10 : Enter Tehsil / Mandal*
Step 7.11 : Enter Post Office
Step 7.12 : Enter Pin Code*
Step 7.13 : Enter District
Step 7.14 : Enter State/UT
Step 7.15 : Enter Date of Birth (if known) (Or) Age [as on 01/01/2018]

Step 11 : Enter Captcha and Click Submit button.

Proofs of date of birth to be attached are as under :
i) Birth certificate issued by a Municipal Authorities or district office of the Registrar of Births & Deaths or Baptism certificate; or
ii) Birth certificate from the school (Govt. / Recognised) last attended by the applicant or any other recognised educational institution; or

iii) If a person is class 10 or more pass, he should give a copy of the marksheet of class 10 , if it contains date of birth as a proof of date of birth; or
iv) Mark sheet of class 8 if it contains date of birth; or
v ) Mark sheet of class 5 if it contains date of birth; or

vii) A certificate of his age given by a sarpanch of the concerned Gram Panchayat or by a member of the concerned Municipal Corporation / Municipal Committee, in case the person is not educated till class 10 and both the parents are not alive.

viii) Indian Passport

x ) Driving License
xi) Aadhar Letter issued by UIDAI
