How to File Form 6?
Offline Application
Just follow the below steps to apply for the Online Voter ID Card.
Steps :
Step 1 : Visit the official website of CEO Assam and click on the “Download Forms” link in the home page.
Step 2 : Then select “Form 6” link and fill the application form with the required details.
Online Application
To apply online you have to visit National Voter’s Service Portal and select the “Apply online for registration of new voter” link
Next fill the required details to complete the registration

(i) I am a citizen of India;
(ii) I am ordinarily resident at the address given in para II above since ………..(date, month, year)
(iii) I have not applied for the inclusion of my name in the electoral roll for any other constituency;
(iv) *My name has not already been included in the electoral roll for this or any other assembly constituency;

*My name may have been included in the electoral roll for_____ Constituency in ___State in which I was ordinarily resident earlier at the address mentioned below and if so, I request that the same may be deleted from that electoral roll.
What is an Electoral roll?
3.1 An electoral roll is a list of all eligible citizens who are entitled to cast their vote in an election. The electoral rolls are prepared Assembly Constituency wise. An electoral roll for any Assembly Constituency is subdivided into parts corresponding with the polling booths.
3.2 To exercise your franchise, the first and foremost requirement is that your name should be in the electoral roll. Without your name registered in the relevant part for the area where you ordinarily reside in the Assembly Constituency, you will not be allowed to exercise your franchise. Therefore, it is your duty to find out whether your name has been registered or not.

As an elector you should immediately check whether your name has been included in the electoral roll of the constituency where you reside or not. You can find out this information from the Electoral Registration Officer of your area. Electoral rolls in all major cities have now been displayed on official websites also.
2. Do you have an Electors’ Photo Identity Card (EPIC)? :
The Election Commission of India has made voter identification mandatory at the time of poll. The electors have to identify themselves with either Electors Photo Identity Card (EPIC) issued by the Commission or any other documentary proof as prescribed by the Commission.

In citizenship certificate my grandfather's name is with the middle name 'mohan' but in other documents like land deed it is 'chandra'. And my grandfather has passed away many years ago. Will it be any problem if we produce documents bearing two different middle name with NRC form?
My father's name appeared in 1966 voter list as harendra brahma and as well as in legacy data. After that period he changed his surname as choudhury and presently his name is appeared in present voter list as Harendra choudhury. He is not alive and I have no evidence of change of surname with me. What I have to do please suggest?
