CEO Haryana Form 6 to Add Name in Electoral Roll
You can obtain a copy of the certified entry by approaching the Electoral Registration Officer (ERO) concerned. You will be required to deposit the requisite fee and thereafter the ERO will issue the required certified copy.
Who can file Form-6?
A. An Indian citizen who has attained age of 18 years or more on the first day of January of the year with reference to which the electoral roll is being revised.
B. A person shifting his / her place of ordinary residence outside the constituency in which he / she is already registered.
Online Registration
Steps :
Step 1 : Visit the official website of CEO Haryana
Step 2 : Scroll the pages down and click on “Online Voter Registration” link.
Step 3 : After that click “Register Online” link. It will redirect to the NVSP portal.
Step 4 : Next Logon to the NVSP website and Click on the Apply online for registration of new voter link.
Step 5 : Fill the application with the required details and upload the necessary documents to complete the registration.
Offline Forms
For inclusion of your name in the Electoral Rolls, fill the following forms:
To download, click on the corresponding form names.
** Form 6 For inclusion of name for the first time and also for inclusion of name of a person already enrolled in a constituency for inclusion of name in another constituency. This form should be submitted to ERO concerned.
** Form 6A Registration of overseas electors.
** Form 7 For objecting inclusion of name in the draft roll and also seeking deletion of name included in the final roll. This form should be submitted to ERO concerned.
** Form 8 For correction to particulars entered in the rolls. This form can be used after draft publications as well as final publication. This form should be submitted to ERO concerned.
** Form 8A For transposition of entry in any other part of the electoral roll of the same constituency. This form can be used after draft publications as well as final publication. This form should be submitted to ERO concerned.
** Form 001A For submission of photograph(0ptional) alongwith form 6/8/8A.
** Form 001B For optional submission of photograph for photo electroral roll & elector photo identity card.
Note :
If any person makes in connection with-
#pd(a) the preparation, revision or correction of an electoral roll, or
(b) the inclusion or exclusion of any entry in or from an electoral roll,
a statement or declaration in writing which is false and which he/she either knows or believes to be false or does not beleive to be true, he/she shall be punishable with imprisonment for a term which may extend to 1 year or with fine, or both.

Who can vote?
All citizens of India who are 18 years of age as on 1st January of the year for which the electoral roll is prepared are entitled to be registered as a voter in the constituency where he or she ordinarily resides. Only persons who are of unsound mind and have been declared so by a competent court or disqualified due to ‘Corrupt Practices’ or offences relating to elections are not entitled to be registered in the electoral rolls.
My wife name is Rekha and apply voter card on 29th June 2014 but not come. Please check and sent back to home
